Serenity Home Supportive Housing is a recovery-centered residential program serving adults in Sonoma County. We provide safe, structured housing for individuals transitioning out of homelessness, substance use treatment, or crisis - offering a stable foundation to rebuild with dignity and purpose.

Our program combines affordable housing with wrap-around support services, peer community, and connection to local resources - because we believe everyone deserves a chance to heal and belong.

Serenity Home accepts referrals from a wide range of community partners, including:

  • Behavioral health units & crisis stabilization programs

  • Case managers & social workers

  • Substance use treatment programs

  • SUD counselors & recovery programs

  • Primary care clinics & community health centers

  • Justice‑system partners (probation, jail re‑entry, diversion programs)

  • Shelters, outreach teams, and homeless service providers

Referring is simple. Complete the referral form with your client’s basic information and needs. Our team reviews all submissions the same day, follows up with you directly, and coordinates a smooth transition into our Santa Rosa home.
